Is the THFC right for me?

Who is aĀ candidate and what to expect when you come to the clinic:

The Transitional Heart Failure Clinic (THFC) is designed to treat and support patients transitioning through phases of heart failure.Ā Our goal is to provide excellent care and quality of life in patients with heart failure. Outside referrals into the clinic require an evaluation by a South Denver Cardiology Associates (SDCA) cardiologist. If the SDCA physician saw you during a hospital stay, then you may be referred directly to the clinic.

If you are unable to participate inĀ Cardiac Rehab, or it is not covered by your insurance company, please ask for alternatives.

The THFC offers a comprehensive program staffed by highly qualified physicians, advanced practice nurses, cardiac R.N.ā€™s, and medical assistants. The first visit is a 1-1/2 hour comprehensive overview that includes a detailed history, physical exam, medication review, and goal setting. Diagnostic and laboratory testing may be ordered. Education is a large focus of the clinic, and the modular education units are available on a link below on this website. Specific areas of educational support include fluid management, diet, exercise, medication management, and psychosocial needs. The clinic can provide same day or next day access and if needed IV diuretics. Once the patient has recovered and has completed the modular education, the patient is transitioned out of the THFC back to their primary cardiologist or physician.

What is CCM Therapy?

CCM stands for cardiac contractility modulation therapy, a treatment that may be an option for people who arenā€™t adequately responding to heart failure medications. CCM is delivered by an implantable pulse generator called an Optimizer that is similar in size to a pacemaker. Learn more about CCM therapy that SDCA offers here in this video.

The Heart Failure Society of America (HFSA) has prepared a series of educational modules designed to help patients learn to live successfully with heart failure. The advice in the modules can help heart failure patients feel better, stay out of the hospital and live longer. The modules are self-contained and written in easy to understand language. Each module provides practical information on a specific topic that is useful to both patients and their families. Each module also includes questions that should ask their doctors or nurses.
