How We Are Making a Difference,

Living with Congestive Heart Failure

When you’re diagnosed with congestive heart failure, it can be frightening. It’s a serious, life-threatening diagnosis that progresses with time. Unfortunately, that can leave some people feeling hopeless and helpless, but it doesn’t have to be that way. Living with congestive heart failure can seem like a lot to take on, but there are a number of ways to make it easier and ensure that your quality of life is as high as possible for as long as possible.

There’s Hope

We don’t want you to spend your days post-diagnosis counting down or wondering when you’re going to go from bad to worse. You can live and live well with congestive heart failure with the right tools, perspective, and therapies. Focusing on that hope, and on living your best life every step of the way is the key to managing this monumental change.

Improving Quality of Life

Our Vision is to improve the quality of care and the quality of life for our patients across the spectrum of heart failure. We strive toward helping our patients to experience progressive healing through the best therapies available. We focus on success without failure. Failure is not an option! Comprehensive Program

Here at South Denver Cardiology, we offer a Transitional Heart Failure Clinic (THFC) designed to treat and support patients transitioning through phases of congestive heart failure. Our goal is to provide excellent care and quality of life in patients with heart failure. Outside referrals into the clinic require an evaluation by a South Denver Cardiology Associates (SDCA) cardiologist. If the SDCA physician saw you during a hospital stay, then you may be referred directly to the clinic.
The THFC offers a comprehensive program staffed by highly qualified physicians, advanced practice nurses, cardiac R.N.’s, and medical assistants. During your first visit, you will spend an hour and a half creating a detailed history, physical exam and medication review to paint the full picture of your experience with congestive heart failure thus far. We will also set goals and may order diagnostic and laboratory testing if it fits your individual needs. The clinic can provide same day or next day access and if needed IV diuretics.

Arming You With Knowledge

Living with congestive heart failure has a lot to do with the knowledge and education you have surrounding your new diagnosis. Education is a large focus of the THFC, and the modular education units are available via a link. Specific areas of educational support include fluid management as edema is one of the most common side effects of congestive heart failure, diet, exercise, medication management, and psychosocial needs.
Understanding what your life is going to look like with congestive heart failure, taking the steps necessary to ensure the highest quality of life with the best therapies and medications throughout the stages of CHF, and knowing that it’s not all over the minute you’re diagnosed are keys to living a good life even as you progress with the disease. If you think THFC might be a good fit for you, reach out to your primary care physician or cardiologist today
