Exercise Treadmill Technician/ Exercise Physiologist (Full Time Position)

Responsibilities:

  • Identifies patients for cardiac stress evaluations
  • Reviews patient records for each day’s schedule.
  • Prepares patients for cardiac stress evaluation by evaluating resting and exercising EKG/blood pressure.
  • Conducts exercising and pharmacological stress evaluations by selecting the appropriate protocol and closely monitors EKG/BP response to stress.
  • Maintains clear communications with physicians, nursing staff, and patient.
  • Has the ability to maintain professionalism when prepping patients who may be partially clothed.
  • Maintains clean and properly functioning work area.

Qualifications/Education:

  • Bachelor’s degree in exercise related program, registered nurse license, EMT certification, or a combination of equivalent experience.
  • BLS certification
  • ACSM certification or comparable experience

Advanced Practice Registered Nursing-APRN

Job Purpose:

Promotes restores, and advocates for patients’ health by means of thorough assessment, evaluation and utilization of evidence-based medicine in collaboration with physicians and multidisciplinary team members; providing compassionate, physical and psychological support to patients, friends, and families.

Skills and Qualifications:

Masters or Doctorate (as of 2015) of Nursing, APRN licensure by the state of Colorado, Prescriptive Authority within the first 12 months of practice, DEA licensure within the first 12 months of practice, Certification by a recognized board, Collaborative Teamwork, Multi-tasking, Listening Skills, Verbal and Electronic Communication, Accurate and Legible Documentation,  ACLS, BLS and professional liability insurance.

Job Description:

  • Updates and organizes inpatient rounding lists daily highlighting consults, discharges, transfers to a higher level of care and procedures. The APRN goals each day include assessing the workload at hand, prioritizing, evaluating the global picture of the teams, and performing, as well as distributing, the work throughout the day.
  • Reports pertinent abnormal findings to a cardiologist.
  • Documents assessment and plan of care in hospital and office records in writing, dictation, and through appropriate electronic communication.
  • Formulates care plans, expands upon differential diagnosis and work up, and initiates orders.
  • Conducts hospital rounds on patients including, but not limited to: Obtaining subjective data including history and review of systems; Obtaining objective data including review of vital signs, telemetry data, laboratory tests, imaging tests, and diagnostic procedures; Perform physical examination; Complete an assessment and formulate a plan of care including initiation of orders; Consults with the cardiologist as needed.
  • Coordinates diagnostic work-ups in conjunction with physicians, health care providers, nursing staff, and other ancillary personnel.
  • Performs detailed history and physical examinations, as well as consultations and formulates a plan of care, consulting with cardiologists when clarification is needed, and ensuring continuity of care.
  • Supervises diagnostic exercise, stress echocardiogram, Thallium, Cardiolyte, Adenosine, Persantine, Lexiscan and Dobutamine treadmill studies, as well as gated blood pool studies and tilt table testing according to established guidelines and standards of care.
  • Evaluates acute and chronic conditions for outpatients in our office setting. Collaborates with care team including, but not limited to MD, RN, MA and supportive services.
  • Interprets test results within their scope of practice.
  • Provides education, support, and problem-solving for patients needing call assistance on weekend daytime hours.
  • Educates patients about their medical conditions including expected course of disease, signs, and symptoms indicating a problem or progression of the disease, when to seek further care, how to access appropriate care and expected and adverse effects of prescribed medications.
  • Collaborates with the healthcare team to determine a plan of care, discharge disposition, and further follow-up including referrals, and utilization of community resources. Communicates with nursing, rehab, device clinic, anticoagulation clinic, amiodarone clinic, and scheduling team for continuity inpatient
  • Participates in research trials through communication, screening, and providing necessary follow up in collaboration with our research department.
  • Maintains patient privacy and protects operations by keeping information confidential.
  • Facilitates the discharge of hospitalized patients including discharge teaching, written and electronic discharge instructions, prescriptions and discharge summary dictation.
  • Writes or electronically transmits prescriptions for patients for acute, self-limiting conditions and for chronic conditions that are stabilized.
  • Teaches wellness classes at the Heart Center for the community. Provides support at group meetings such as Walk with a Doc, Women and Heart Disease, Pulmonary Hypertension support group, or AICD support group.
  • Maintains professional, cooperative decorum and promotes collaborative, positive relationships with colleagues, physicians, referring health care providers, ancillary staff, administrative staff, patients and their families on behalf of South Denver Cardiology.
  • Supervises learning experiences of selected students and other personnel. Participates in continuing education for self, other APRNs, and staff members. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
  • Remains current in required professional and credentialing status. Maintains current BLS, ACLS, professional insurance, DEA, and state licensure, professional certification, education requirements for JACHO and professional certification, tuberculosis testing, immunizations, HIPPA and OSHA standards. Provides appropriate documentation to credentialing specialists in a timely manner.

Assures quality of care by adhering to standards set forth by professional organizations, state board of nursing, Colorado Nurse Practice Act, credentialing committees, community standards of care, payer regulations and SDCA requirements.

Meets with APRN team members monthly for education opportunities, practice updates, and collaborative schedule agreements.

Cardiac Rehab Registered Nurse-Full Time Position

Responsibilities:

  • Identifies patient care requirements by establishing a personal rapport with potential and actual patients
  • Gathers baseline conditioning/cardiac status through current treadmill evaluations
  • Conducts group orientation class to further inform patients and their families of cardiac rehab environment
  • Monitors telemetry during cardiac rehab classes
  • Establishes a compassionate environment by providing emotional and psychological support in a professional manner to patients and families.
  • Gathers current medication list and ensures patients fully understand the reason for and actions of all medications
  • Contacts physician if indicated to modify medication regime
  • Assures quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals, and making or recommending necessary adjustments

Qualifications/Education:

  • Registered Nurse with a current Colorado License, BLS/ACLS, critical care experience preferred

RN/Clinical Nurse (Full Time Position)

Responsibilities:

  • Must maintain patient confidentiality and privacy per current HIPAA regulations.
  • Responsible for triaging patients, by phone or in person. Must incorporate the use of established protocols, critical thinking, prioritizing, and delegation to resolve patient needs.
  • Oversees the communication of all prescription requests by confirming with Physician and the pharmacy of accuracy.
  • Will educate patients on their medical conditions, symptoms, and the possible effects or adverse reactions that may occur with medications.
  • Review diagnostic test results with physicians.  Differentiates normal results from abnormal results and obtains recommendations from the Physician on next steps.  Informs patient of results and recommendation.
  • Routinely see patients in the clinic for blood pressure management, medication titration, arrhythmia management, and congestive heart failure exacerbations.
  • Will assist in the management of Warfarin patients per protocol.
  • Documents all patient care activities in patient record.
  • Assists in pre and post procedure education.
  • Must be equipped in gaining patient cooperation by reducing anxieties, providing explanations of procedures, and answering questions.
  • Acts as a resource for the medical assistants and non-clinical staff.
  • Assists the diagnostic team in administering pharmacological agents for diagnostic testing. Provides routine and emergent care as directed.
  • Is an active member of code blue team. Responsibilities include BLS/ACLS protocols.

Clinical Medical Assistant (Full Time Position)

Responsibilities:

  • Must maintain patient confidentiality and privacy per current HIPAA regulations.
  • Must be able to establish rapport with patients easily and be customer service focused.
  • Needs to be able to communicate so as to reduce anxieties, providing explanations of treatment and answering questions for a variety of patients.
  • Assists patients and physicians by escorting patients from the lobby to exam rooms, obtaining vital signs, getting medication list and preparing patients for examination.
  • Obtains diagnostic data by performing point of care testing/treatments such as protimes, EKGs, and pulse oximetry.
  • Performs clerical duties such as checking patients in and out, filling out disability forms, correcting/editing dictation inaccuracies, managing charts and obtaining physician signatures.
  • Complies with federal, state, local, and professional requirements to keep licensing in good standing and by advising management of needed updates.
  • Performs EECP once trained and certified.

Qualifications/Education:

Health Information Professional – Medical Records  (Full Time)

RESPONSIBILITIES:

• Must maintain patient confidentiality and privacy per current HIPAA regulations
• Ability to read and decipher various types of medical reports to ensure the proper reports get scanned, electronically filed/dated, and tasked to the appropriate MD within EMR as necessary
• Ability to fluidly navigate through our Allscripts scheduling system, as well as our Allscripts EMR system and EMR scanning system
• Responsible for preparing patient office charts for all types of scheduled office visits
• Have the ability to break down and scan paper charts into EMR
• Will assist in interacting with patients and outside entities requesting medical records and as such, completing medical records requests as necessary
• Assist with any additional duties requested by the Medical Records Supervisor

QUALIFICATIONS/EDUCATION:

• Ability to be self-motivated, detail-oriented, and have the ability to multitask, problem solve and prioritize tasks in a fast-paced environment
• Requires excellent knowledge and high usage of computers and medically-based computer applications
